Virtual Intake Appointment
What is the virtual intake appointment and how do I book one?
In order to join the classes offered by the BC Brain Wellness Program, please register below to book a virtual intake appointment. These are orientation and safety screenings done virtually over video. Their purpose is to help determine an appropriate level of activity for you, identify classes that may be a good fit for your interests and needs, and help us understand how our program can best support you. Please note that a virtual intake appointment is required before participating for all participants. If you are a returning participant and have not completed one already, please book for one as well.
To book an appointment, please follow these steps:
Click on the button at the bottom of the page for the type of virtual intake appointment you’d like to book for. This will take you to our booking page.
Select the date and time that works best for you.
You will then be prompted to create a Jane account with the BC Brain Wellness Program. Please do this. If you have an existing account with us, you can log in instead.
Fill in the Consent and Intake Form. If you are unable to complete this form, please inform your assessor during your appointment and they will review it with you.
Below are the types of virtual intake appointment types we offer:
Exercise & Creative Classes - Virtual Intake: Please book this type of appointment if you plan on participating in our online exercise and/or creative classes.
ONLY Creative Classes - Virtual Intake: Please book this type of appointment if you plan on participating in creative/non-exercise classes only.
Long COVID - Virtual Intake: Please book this type of appointment if you are new to the program, and are experiencing Long COVID symptoms.
Follow-up Virtual Reassessment: Please book this type of appointment if you have previously done a safety screening and are looking to be reassessed.
The entire virtual intake process is done on Jane: a secure and encrypted telehealth platform. Once you have booked your appointment, you will be emailed instructions the day before on how to access the video call.
Please note BC Brain Wellness Program classes are intended for residents of British Columbia only.
What to expect during the “Exercise & Creative Classes – Virtual Intake” session:
The virtual intake appointment will take roughly 45 - 60 mins. You will chat about your current level of activity and be asked to perform some basic testing of balance and strength (e.g. getting up from a chair). This allows us to make sure your workout area is nice and safe, allows us to recommend classes that might work best for you, and aids with the research side of the program.
You will require some light equipment:
Straight back chair with no armrests (if possible)
Tape measure
Tape (to stick on to wall/floor for marking)
Yoga mat or carpeted area
Weights
We strongly suggest using a computer or laptop with a camera for this appointment. If you have technical difficulties, please let us know.
What to expect during the “ONLY Creative Classes – Virtual Intake” session:
The virtual intake appointment will take roughly 30 mins. You will chat about your current level of activity, personal interests and goals with the program. This allows us to recommend classes that might work best for you.
No equipment is required during the session.
We strongly suggest using a computer or laptop with a camera for this appointment. If you have technical difficulties, please let us know.
Privacy Notification: When you book an assessment, your personal information is collected under the authority of section 26(c) of the Freedom of Information and Protection of Privacy Act (FIPPA). This information will be used to assess, treat, and make appropriate recommendations for you. Questions about the collection of this information may be directed to brain.wellness@ubc.ca.
