Discover Self-Management BC: Free Support for Living Well with Chronic Conditions

Living with ongoing or long-term health conditions can bring daily challenges – pain, depression, fatigue, anxiety, mobility, social isolation, accessing health care, and many other physical and emotional difficulties are common. It can be overwhelming, while still trying to accomplish the things you need to do and things you enjoy. Join us for an informative discussion about Self-Management BC (SMBC) and its evidence-based support programs, provided in BC at no cost for adults of all ages with chronic physical and/or mental health conditions. Interested persons can choose the type of support that works best for them, with options from 6-session small-group workshops available in-person, virtually, or online, to weekly one-to-one coaching by telephone for three months.

Participants gain practical tools and develop skills to better manage their health and build confidence to live well. Come learn more, hear real-life experiences, and discover whether a Self-Management Program could be a good fit for you.
